| The Registrar of Vital Statistics is charged
with recording, maintaining, and making available all records of birth,
marriage, deaths, and domestic partnership occurring in Freehold Borough.
Copies of the following records may be obtained from the
Registrar's Office if the birth, death, marriage, or domestic partnership occurred in Freehold
Borough:

Marriage License |

| If the bride is a
Freehold Borough resident, a couple applies for their Marriage License in
Freehold Borough at the Registrar’s Office, 51 West Main St.
Monday - Friday -8:30 a.m. - 4:00 p.m. - Please call to make appointments.
The bride, groom and a witness 18 years of age or older, who knows the
bride and groom, must appear together at the Registrar’s Office to apply for
the marriage license.
The bride and groom must bring the following information:
Proof of Residence of Bride
Social Security Cards
Raised seal birth certificate of the bride and groom.
$28.00 fee payable in cash or by money order.
If the bride or groom has been married before, a raised seal divorce
decree or death certificate is required when applying for the new marriage
license.
If either the bride or groom is under 18 years of age, call the Registrar

Obtaining Birth, Death
or Marriage Record |

| In accordance with
the Governor's Executive Order #18, only the following persons are entitled
to a certified copy of a vital record: subject, subject's parent, legal
guardian or legal representative, spouse, child, grandchild or sibling, if
of legal age. In order to receive a certified copy of a birth, death, or
marriage record, you must complete the application(s) below and send the
following information in with your request:
A copy of a photo ID showing your address – OR
On a photo ID without address, you must submit one other form of ID
showing address – OR
Two alternate forms of ID showing address, i.e. vehicle registration,
insurance card, voter registration, passport, green card, county ID, school
ID, utility bill.
Fee of $10.00 in cash or money order made payable to Freehold Borough. No
personal checks will be accepted. Change as of June 1st, 2006.
